Call toll-free 1-800-477-6472 Monday through Friday between 9:00 am and 8:00 pm (et).

You will reach a Reimbursement Associate who will answer your questions. If all Associates are busy, you will be asked to leave your name and telephone number. An Associate will contact you within one business day.

The primary purpose of the AmBisome Reimbursement Hotline is to provide information that helps you understand insurer's policies. Astellas Pharma US, Inc. and the Hotline do not formally make appeals for Hotline callers, nor do we guarantee that you will be successful in obtaining insurance payments. Third-party payment for medical products and services is affected by numerous factors, not all of which can be anticipated or resolved by the Hotline staff.

Offering A Range of Reimbursement Assistance

Reimbursement Associates for AmBisome can:

• Contact insurance companies to determine a patient's benefits for AmBisome.
• Provide information on billing codes and claims submission.
• Educate insurers about AmBisome. If you identify an insurer unfamiliar AmBisome, the Hotline will contact the insurer and send materials to inform the insurer about the drug.
• Help resolve denied claims, analyze the reason for a claim denial, and make recommendations for an appeal.

When you call, please have the following information available:

• Provider number
• Date of Service
• Patient's name and ID number
• Insured's group number
• Name and address of insurer
• Insurer's reason for rejecting the claim
• A copy of the claim and explanation of benefits (EOB) form.
